Why do we require your personal information?
The personal information collected and maintained by Affinity Insurance Brokers comprises your name, address, contact details and information specific to your product preference.
Depending on the type of policy or quotation you apply for or have or the type of claim you make, we may also collect and update personal information necessary to set up and maintain the policy, such as date of birth, bank account details, postal address and the address to base the risk, previous insurance history, and other information about your driving history. We also record information about the status of your policy and claim and the alterations you make or wish to make on your policy and the claims you have made.
For some policies and claims we also collect and maintain your sensitive information. This comprises information about your health, membership of professional associations or the like and any criminal convictions you may have.
The information requested allows us to assess risks, write and administer your insurance cover and manage claims made by you or against you.
Disclosure of your personal information to third parties
We disclose personal information to third parties who we believe are necessary to assist both us and you, in providing all relevant services and products. For example, in handling claims, we may have to disclose your personal and other information to third parties such as other insurers, loss adjusters, investigators, agents and others involved in the claims handling process, or as required by law. We limit the use and disclosure of any personal information provided by us to them to the specific purpose for which we supplied it.
Contacting us about access and correction of your personal information
Affinity Insurance Brokers aims to ensure that your personal information is accurate, up to date and complete. Please contact us on (03) 8587 7777 between 9am - 5pm on Monday to Friday, if you would like to seek access to, or revise your personal information or feel that the information we currently have on record is incorrect or incomplete or you believe that the privacy of your personal information at Affinity Insurance Brokers has been interfered with. If this should occur you are entitled to raise your concerns. Your complaint will be managed and resolved through our internal Privacy Complaint Procedure.

From time to time we may advise or offer you information on other Affinity Insurance Brokers products or services that may be relevant and of interest to you.
